Medical Records Access

 

 

In compliance with regulations of the Occupational Safety and Health Administration, it is the policy of UNC Charlotte that all employees who are exposed to some type of potentially toxic substance or harmful physical agent have a right of access to relevant exposure and medical records.

UNC Charlotte must inform its employees annually of:

  1. The existence, location and availability of any records covered by the standard.  Medical records include medical questionnaires or histories, the results of pre-employment or periodic medical examinations, medical opinions and recommendations, descriptions of treatments and prescriptions, or employee medical complaints.  Exposure records include noise or sound level surveys and any other results of sampling for any vapors or dusts defined as toxic or harmful.

  1. The person responsible for maintaining and providing access to these records.

  1. Each employee's right to access to these records upon request.

This policy applies only to those employees who are exposed
to some type of potentially toxic or harmful physical agent.

  1. Access to records must be made within fifteen (15) days after the request.

  1. Medical records must be preserved for duration of employment plus thirty (30) years.

  1. Exposure records and analysis of exposure or medical records must be preserved for thirty (30) years.
